Submitting your information here is
NOT the same as filling out the Information
Sheets that Mr. Zimmer sends to the theater companies. This
information is being used ONLY for posting on
this website. Continue to fill out and send in the forms from Mr.
Zimmer.

Fields marked with an asterisk (*) must
be filled in. So if, when you get to the leading and supporting
fields, the show you are submitting information for does not have
anyone to list in one of the categories (ie. Nunsense might
not have any names for Leading or Supporting Actor), please type N/A
or "none" in those fields.

If you have more than one leading or
supporting actor/actress, hit ENTER after each name to add another.

A separate form must be filled in for
each show you with to submit information for. After clicking the
SUBMIT button, click the [BACK] link on the page that loads. That
will take you back to a new form.

Choreographer and Costumer have separate
fields from the rest of the designers only because they are the only
positions besides Director and Music Director to have their own
awards. If (when?) the other designers get their own awards, those
will be given their own fields here.

The field for "Non-Performing Person's
Specialty" is for informational purposes only, so that people who
may have never heard of someone nominated can be made aware of the
nature of that person's contribution. As name are suggested for
Non-Performing Person, I will be adding separate page to list those
names and their specialties.

I ask for the name and email address of
the person submitting the form, as well as the name and email
address of the theater company's producer, so that if I get more
than one submission for a show, I know who submitted them and can
clear up any confusion.
